Free Hospitalization Program

Provides medical and hospitalization assistance to qualified indigent residents of Barangay Tabid through barangay endorsement and inter-agency coordination.

Requirements

  • Certificate of Indigency (from Barangay)
  • Barangay Clearance
  • Proof of Residency
  • Valid ID of patient or representative
  • Hospital bill / Medical abstract
  • PhilHealth card (if member)
  • Endorsement letter from Barangay

Process

  1. Visit the Barangay Hall and request Certificate of Indigency and Endorsement Letter
  2. Submit documents to the Municipal Social Welfare Office
  3. Proceed to the Provincial/District Hospital Billing Office
  4. Present all documents for verification
  5. Receive hospital bill discount or waiver

Eligibility

  • Registered resident of Barangay Tabid
  • Classified as indigent or low-income
  • Currently admitted or recently discharged
  • Not covered by other full-assistance programs
  • Senior citizens and PWDs given priority

Social Welfare Assistance

Provision of emergency financial aid, livelihood support, and social protection services to qualified residents in need.

Disaster Relief & Emergency Assistance

Rapid response program providing relief goods, evacuation support, and rehabilitation assistance during and after disasters or calamities.

  • Relief goods (food packs, water)
  • Temporary shelter and evacuation assistance
  • Emergency financial assistance
  • Non-food relief items (hygiene kits)
  • Post-disaster rehabilitation support

How to Avail

  1. Register at the Barangay Hall or evacuation center
  2. Present valid ID or any proof of residency
  3. Receive acknowledgment receipt
  4. Claim relief goods at designated distribution point

Who Can Avail

  • All affected residents of Barangay Tabid
  • Evacuees registered at barangay evacuation center
  • Priority: Elderly, PWDs, pregnant women, children
